Instructions:

Please provide the requested information regarding your commercial guide trips for the reporting period.

As per Title 301 Chapter 2 Regulation 030 of the Kentucky Administrative Regulations, please note the following reporting timeframe requirements:

• Fishing Guide Reports: Submit by the 15th day of every month for the previous month's guiding activities, if "I plan to be a fishing guide" or "Both hunting & fishing guide" is indicated on your Commercial Guide License Application Form, even if no guiding occurred.
• Hunting Guide Reports: Submit within thirty (30) days after the closure of the hunting season for any species for which commercial guiding occurred.
• Year-Round Hunting Species: For species that can be hunted year-round, submit reports within thirty (30) days after the end of the license year which is the last day of February.

Note on Bowfishing: Bowfishing trips should be reported under the fishing guide reporting form.
